Care Mapping for Adults with Phenylketonuria (PKU): A Pilot Initiative to Reframe Needs
Description
As individuals with PKU transition into adulthood, continuity and coordination of care often diminish, leading to unmet medical, psychosocial, and nutritional needs. A care map is a custom, visual depiction of an individual’s life. The process of care mapping, hand drawing a depiction of required support and services, has been used for other complex health conditions to reframe needs by identifying treatment gaps and missing connections across systems. The purpose of this workshop is to share the PKU Care Map results and lessons learned with the goal of motivating their use in clinical and professional settings.
This workshop occurs on Thursday from 1:30 - 2:30pm and is repeated from 3:30 - 4:30pm.
